GENERAL MEETING INFORMATION
Course Description
The IUA provides a quality support system for the urologic
physicians in Indiana, and those practicing this specialty in
surrounding environs adjacent to the state of Indiana, in the
efforts to better serve patients, people and the community areas.
The 2012 Scientific Program will offer presentations on topics
of interest to urologists including treatments and advances
regarding stress urinary incontinence, bladder cancer and
robotic prostatectomy, as well as changes in residency training.
The socioeconomic session will feature a legislative update
which will be of interest to all those who practice urology in the
state of Indiana.

Room Rate
IUA has negotiated a rate of $159.00 plus tax. This rate is for single or double occupancy in a standard room at The Westin Indianapolis.
Hotel Deadline
The IUA discounted rate is based on availability and must be booked by March 28, 2012. The block may sell out prior to this date; therefore attendees are strongly encouraged to make reservations early. If the IUA block sells out, or if you make reservations after March 28, 2012, you will be charged the hotel’s standard rate.

Check-In/Check-Out
Check-in time is 3:00 p.m. and check-out time is 12:00 p.m. Please contact the hotel directly if you have early check-in or late check-out requests.
Additional Charges
The guest room rate of $159.00 is subject to prevailing state and local taxes, currently 17%.
Cancellation Policy
If you wish to cancel, please do so 72 hours before arrival to avoid cancellation penalties.

Rental Car Information
Avis Rent-A-Car is the official rental car company for the 2012 IUA Annual Meeting. For reservations, please call (800) 331-1600. You must mention the code “J901055” to receive the discounted rates.
Taxi Cab Services
Taxi service is available at the airport on the lower level of the terminal just outside Baggage Claim. There is a $15 minimum charge for all fares from the airport, regardless of distance. If you need to travel outside the Indianapolis area, consult your driver for rates. Fares may not exceed the amount shown on the meter. Please tip drivers appropriately for their service.

Parking
The Westin Indianapolis offers both Self Parking ($26 per day) and Valet Parking ($32 per day with in and out privileges). Self parking does not offer in and out privileges and guests are required to pay at the parking lot kiosks, which are located on the first and second floors of the garage. Valet parking offers in and out privileges and the parking is charged to the guest’s room.

Early Registration Deadline
Meeting registration forms must be received by the IUA Executive Office by 4/13/2012 to receive the discounted early registration rate. All forms received after this deadline will be assessed the regular registration fee.
Registration Cancellation & Refund Policy
Registration refund requests must be submitted in writing to the IUA Executive Office by 4/13/2012. All refund requests will be subject to a $25.00 processing fee. No refunds will be made after 4/13/2012.
